Distance Learning Option Form

In light of the COVID-19 health emergency and in accordance with the District’s 2020-21 Reopening Plan filed with the New York State Education Department, I am voluntarily requesting the option of a Remote Learning Model only for the 2020-21 school year. I acknowledge that I have read the District’s Reopening Plan which is located on the District’s webpage. I understand the District has offered the Hybrid Model as the District’s recommended model and that I am declining that option. I understand that the Remote Learning Model for my child will consist of the following (see chart below):
I acknowledge that in electing the Remote Learning Model for my child, I do so with the understanding that my child will need to continue with the Remote Learning Model for at least the first quarter of the school year as a result of the District’s need to properly plan for in-person instruction. I further acknowledge although the District will make reasonable efforts to grant my request to change my election, in-person instruction for my child may no longer be an option for the 2020-21 school year once I sign this election form since classrooms and in-person schedules were configured in a socially distant manner to accommodate a certain number of students. Should I subsequently decide to request in-person instruction for my child during the 2020-21 school year, I will submit a written request to the office of the Superintendent of Schools. Written request to change your election must be made as follows: Friday, October 30 for the second quarter; Friday, January 15 for the third quarter and Friday, April 9 for the fourth quarter. However, I understand and acknowledge that this request may not be granted and shall only be granted at the discretion of the Superintendent of Schools, based upon instructional and other factors as determined by the Superintendent. Please complete this form no later than Friday, August 21. You will receive a letter in the mail as well. You do not need to complete both the letter and this form.
